Living Will (Canada)
  Living Will form that states that should there come a time that the signor can no longer communicate, and if it is the opinion of two independent doctors there is no reasonable prospect of recovery, that artificial means of life support not be used. This form can be used in the following provinces: Alberta, British Columbia, Manitoba, New Brunswick, Newfoundland and Labrador, Northwest Territories, Nova Scotia, Nunavut, Ontario, Prince Edward Island, Saskatchewan and Yukon. |
