Depiction Release - Documentary (Short Form_
| This short-form release allows a Producer to record an interview of a subject for inclusion in a motion picture or television documentary. For use in all states. |
California Depiction Release - Life Story with Reversion
| This release grants a producer the rights to a person's life story to be used for a motion picture or television program. If the production does not commence within an agreed period of time, the rights revert back to the subject. For use in all California. |
Depiction Release- Life Story (Option Purchase Format)
| This release grants a producer the right to film an individual in anticipation of a documentary motion picture or television program. The producer pays for the option to produce the story for a period of time. If that option is used by the producer, the subject will receive additional compensation. For use in all states. |
Permission to Film Presentation
| This form allows the person who is giving a presentation to give permission to a producer (can be an individual or company) to film that presentation for a specific purpose (e.g. training video). This form includes provisions which prohibit the producer from using the film in any other way or selling it. |
Performing Agreement - Theater Group
